SmartCare PC2 provides real-time phone access for primary care providers to consult with a specialist in behavioral health care. The goal of the program is to support primary care providers in managing behavioral health concerns of their patients by providing access to psychiatric consultations.
Consultations through SmartCare PC2 may include:
- differential diagnosis of mental health conditions
- efficacy and use of particular psychotropic medications over the course of the patient’s treatment
- availability of local resource support

SmartCare PC2 is available 8:30 to 4:30 for any provider of health care in the FQHC systems throughout the county of San Diego.
We will also link to articles of interest in the wide variety of subjects associated with psychiatry and the practice of primary care medicine.

The nature of this program does not allow for direct interaction between patient and a SmartCare PC2 mental health consultant or the ability to assess or independently diagnose a patient’s condition. SmartCare PC2 relies on the information provided by the primary care provider to offer guidance, information and advice. The FQHC primary care provider retains responsibility for follow-up care, medication management, and referrals as needed. Follow-up consultations will be available on request of the primary care provider to support ongoing care as needed.
Patients whose care is supported through an existing “pairing” with a County funded mental health clinic or those who are treated by a FQHC clinic psychiatrist will preferentially be served by these providers as the primary source of consultation. SmartCare PC2’s consultations are available as a secondary resource when a “paired” county program psychiatrist or the treating FQHC clinic psychiatrist is not available.
