ACE-Adverse Childhood Experiences The survey measures 10 types of childhood trauma; five are personal — physical abuse, verbal abuse, sexual abuse, physical neglect, and emotional neglect; five are related to other family members: a parent who’s an alcoholic, a mother who’s a victim of domestic violence, a family member in jail, a family member diagnosed with a mental illness, and the disappearance of a parent through divorce, death or abandonment. Each type of trauma counts as one.


AIMS-Abnormal Involuntary Movement Scale is a clinician administered test to record the occurrence of tardive dyskinesia in patients receiving neuroleptic medications.

CAGE is a commonly used, 5- question tool used to screen for drug and alcohol use. It is a quick questionnaire to help determine if an alcohol assessment is needed. If a person answers yes to two or more questions, a complete assessment is advised.

PHQ -2  Patient Health Questionnaire 2 (PHQ-2) is used to screen patients for undiagnosed depression.

PHQ-9 is a depression scale consisting of nine questions. The PHQ-9 can be used as a tool for diagnosing depression as well as for monitoring the patient’s treatment.

Screen for Child Anxiety Related Disorders-SCARED  A child and parent self-report instrument used to screen for childhood anxiety disorders including general anxiety disorder, separation anxiety disorder, panic disorder, and social phobia. In addition, it assesses symptoms related to school phobias.

Spence Children’s Anxiety Scale-SCAS

The Spence Children’s Anxiety Scale was developed to assess the severity of anxiety symptoms broadly in line with the dimensions of anxiety disorder.  There is a screening tool for children and for the parent/caregiver.



Vanderbilt The Vanderbilt Assessment Scales are to help diagnose ADHD in children between the ages of 6 and 12 years.


Yale Brown Obsessive Compulsive Checklist Y-BOCS This self-rating scale is designed to assess the severity and type of symptoms in patients with OCD.
